Board-certified neurologist and psychiatrist

A score is not a diagnosis.

You see the changes first: the lost word, the repeated question, the medication left on the counter. Dr. Jonathan Mueller, co-developer of Cognistat, created the CHAMP Medical Portrait to turn what you notice into a clear picture and the right questions for your doctor. It is free.

Why a portrait

Memory is many things. A single score can hide which parts of thinking are changing, and what might explain it.

MemoryAttentionLanguagePlanning and judgmentDaily activitiesSleepMood and stressMedications
01

You carry it all

You keep track of who said what, what changed, and what to tell the doctor. The details scatter between visits.

02

Visits are short

A busy appointment rarely has room for the whole story. A clear portrait makes the time count.

03

Better questions

Knowing which areas are changing helps you ask what matters, and plan ahead as a family.

Step one, free

The CHAMP Medical Portrait.

A guided set of questions about thinking, daily life and health. Family members can add what they see through the CHAMP Caregiver Interview, so the picture includes the people who know you best.

What you receive, free

  • Your CHAMP Medical Portrait
  • A personalized list of questions to take to your physician
  • Plain-language explanations of what each area means

Built on his research

The portrait applies the approach Dr. Mueller proposed in Alzheimer's & Dementia in 2023: look at each area of thinking on its own instead of relying on one sum score.
